A phrase that comes up often in fertility care, usually said gently and with the best intentions is, Everything looks normal. For some patients, that's a relief. For others, it's the start of a more complicated chapter, the one where the standard tests didn't find anything but the trying still isn't working. That gap, between the words on the chart and the reality of what's happening, is what we wanted to spend time with this week.

Both features in this edition take it seriously. The first looks at heart rate variability as an emerging fertility-relevant signal that standard panels don't measure. Heart rate variability, the tiny variation in time between consecutive heartbeats, has become one of the most studied biomarkers in sports medicine and cardiology. Putting the Pieces Together

What does it actually mean when your fertility workup comes back normal? Less than you'd think. The standard panel checks for a specific set of things: mostly hormonal, anatomical, and male-factor basics. Anything outside that list, including factors with real research behind them (thyroid antibodies, sperm DNA integrity, the endometrial microbiome, metabolic markers, sleep), doesn't get measured unless someone specifically asks. This week's second feature is the field guide to what's on the standard panel, what's not, and how to talk to your doctor about the gap.
